Maddesindeki ilkelere uygun olarak ki\u015fisel veri i\u015f faaliyetlerinde bulunmaktad\u0131r. <\/span><span style=\"vertical-align: inherit;\">Hastane kanunlarda veya ki\u015fisel e\u015fyalar\u0131n amac\u0131n\u0131n s\u00fcresi kadar ki\u015fisel e\u015fyalar <\/span><span style=\"vertical-align: inherit;\">Sa\u011fl\u0131kta kazan\u0131labilecek niteliktedir. <\/span><span style=\"vertical-align: inherit;\">Ki\u015fisel, verilen sa\u011fl\u0131k hizmetlerine ba\u011fl\u0131 olarak de\u011fi\u015febilmekle birlikte otomatik ya da otomatik olmayan web sitesi, \u00e7a\u011fr\u0131 merkezi, internet hizmetleri ve benzeri ara\u00e7larla s\u00f6zl\u00fck, kay\u0131tl\u0131 ya da elektronik olarak toplanmaktad\u0131r.<\/span><\/span><\/span><\/p>\n<p><span style=\"color: #000000;\"><strong><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">Ki\u015fisel Verileriniz;<\/span><\/span><\/strong><\/span><br>\n<span style=\"color: #000000;\">Ad\u0131n\u0131z, soyad\u0131n\u0131z, TC Kimlik numaran\u0131z, pasaport numaran\u0131z veya ge\u00e7ici TC Kimlik numaran\u0131z, do\u011fum yeri ve tarihiniz, medeni haliniz, cinsiyetiniz, hastanemize \u00f6zg\u00fc hasta numaran\u0131z ve sizi tan\u0131mlayabilece\u011fimiz di\u011fer kimlik verileriniz; adresiniz, telefon numaran\u0131z, elektronik posta adresiniz ve sair ileti\u015fim verileriniz, \u00f6deme ve faturalama bilgileriniz gibi finansal verileriniz; muhtelif zamanlarda taraf\u0131m\u0131zca elektronik olan veya olmayan y\u00f6ntemlerle edinilebilecek sesli ve dijital bilgiler; Hastanemizi ziyaret etmeniz halinde kamera kay\u0131tlar\u0131 g\u00f6r\u00fcnt\u00fcleriniz; t\u00fcm t\u0131bbi te\u015fhis, muayene, tedavi ve bak\u0131m hizmetlerinin y\u00fcr\u00fct\u00fclmesi s\u0131ras\u0131nda elde edilen ki\u015fisel sa\u011fl\u0131k verisi ba\u015fta olmak \u00fczere genel ve \u00f6zel nitelikli ki\u015fisel verileriniz; sa\u011fl\u0131k hizmetlerinin finansman\u0131 ve planlamas\u0131 amac\u0131yla \u00f6zel sa\u011fl\u0131k sigortas\u0131na ili\u015fkin verileriniz ve Sosyal G\u00fcvenlik Kurumu verileriniz, internet sitemiz \u00fczerinden g\u00f6nderdi\u011finiz sa\u011fl\u0131k ve kimlik verileriniz \u00f6deme ve faturalama bilgileri gibi finansal verileriniz dahil olmak ve bunlarla s\u0131n\u0131rl\u0131 olmamak \u00fczere verileriniz hastanemiz taraf\u0131ndan i\u015flenecektir.<\/span><\/p>\n<p><span style=\"color: #000000;\">Ayr\u0131ca, \u0130stanbul Hospital online hizmetlerini kullanmak amac\u0131yla \u00e7a\u011fr\u0131 merkezimizi veya internet sitemizi kulland\u0131\u011f\u0131n\u0131zda , internet sayfalar\u0131n\u0131 ziyaret etmeniz ya da bu sitelerde gezmeniz halinde de ki\u015fisel verileriniz i\u015flenebilecektir.<\/span><\/p>\n<p><span style=\"color: #000000;\">Ki\u015fisel verileriniz, KVK Kanunu\u2019nun 5. ve 6. maddelerinde belirtilen ki\u015fisel veri i\u015fleme \u015fartlar ve ama\u00e7lar\u0131 dahilinde i\u015flenecektir. Sa\u011fl\u0131k verileriniz ba\u015fta olmak \u00fczere \u00f6zel nitelikli ki\u015fisel verileriniz ve genel nitelikli ki\u015fisel verileriniz, Hastanemiz taraf\u0131ndan a\u015fa\u011f\u0131da belirtilen ama\u00e7larla i\u015flenebilecektir;<\/span><\/p>\n<p><span style=\"color: #000000;\"><strong>\u00d6zel nitelikli ki\u015fisel verileriniz de d\u00e2hil olmak \u00fczere ki\u015fisel verileriniz,<\/strong><\/span><\/p>\n<ul>\n<li><span style=\"color: #000000;\">3359 say\u0131l\u0131 Sa\u011fl\u0131k Hizmetleri Temel Kanunu, 663 say\u0131l\u0131 Sa\u011fl\u0131k Bakanl\u0131\u011f\u0131 ve Ba\u011fl\u0131 Kurulu\u015flar\u0131n\u0131n Te\u015fkilat ve G\u00f6revleri Hakk\u0131nda Kanun H\u00fckm\u00fcnde Kararname,<\/span><\/li>\n<li><span style=\"color: #000000;\">\u00d6zel Hastaneler Y\u00f6netmeli\u011fi, Ki\u015fisel Sa\u011fl\u0131k Verilerinin \u0130\u015flenmesi ve Mahremiyetinin Sa\u011flanmas\u0131 Hakk\u0131nda Y\u00f6netmelik ve ilgili di\u011fer d\u00fczenlemelerde yer alan hukuki y\u00fck\u00fcml\u00fcl\u00fcklerimizi yerine getirme;<\/span><\/li>\n<li><span style=\"color: #000000;\">\u00d6zel sa\u011fl\u0131\u011f\u0131n korunmas\u0131, koruyucu hekimlik, t\u0131bb\u00ee te\u015fhis, tedavi ve bak\u0131m hizmetlerinin y\u00fcr\u00fct\u00fclmesi,<\/span><\/li>\n<li><span style=\"color: #000000;\">Sa\u011fl\u0131k hizmetleri ile finansman\u0131n\u0131n planlanmas\u0131 ve y\u00f6netimi;<\/span><\/li>\n<li><span style=\"color: #000000;\">Randevu alman\u0131z halinde randevu hakk\u0131nda sizi haberdar edebilme;<\/span><\/li>\n<li><span style=\"color: #000000;\">Hastanemizin i\u00e7 i\u015fleyi\u015finin planlanmas\u0131 ve y\u00f6netilmesi,<\/span><\/li>\n<li><span style=\"color: #000000;\">Su istimal ve yetkisiz i\u015flemlerin izlenmesi ve engellenmesi;<\/span><\/li>\n<li><span style=\"color: #000000;\">Risk y\u00f6netimi ve kalite geli\u015ftirme aktivitelerinin yerine getirilmesi;<\/span><\/li>\n<li><span style=\"color: #000000;\">Ara\u015ft\u0131rma yap\u0131lmas\u0131;<\/span><\/li>\n<li><span style=\"color: #000000;\">Hizmetlerimiz kar\u015f\u0131l\u0131\u011f\u0131nda faturaland\u0131rma yapma;<\/span><\/li>\n<li><span style=\"color: #000000;\">Kimli\u011finizi do\u011frulama;<\/span><\/li>\n<li><span style=\"color: #000000;\">Hastanemiz ile anla\u015fmal\u0131 olan kurumlarla ili\u015fkinizi teyit etme;<\/span><\/li>\n<li><span style=\"color: #000000;\">Sa\u011fl\u0131k hizmetlerinin finansman\u0131 kapsam\u0131nda \u00f6zel sigorta \u015firketler ile talep edilen bilgileri payla\u015fma;<\/span><\/li>\n<li><span style=\"color: #000000;\">Sa\u011fl\u0131k hizmetlerimize ili\u015fkin her t\u00fcrl\u00fc soru ve \u015fik\u00e2yetinize cevap verebilme;<\/span><\/li>\n<li><span style=\"color: #000000;\">Hastanemiz sistem ve uygulamalar\u0131n\u0131n veri g\u00fcvenli\u011fi kapsam\u0131nda t\u00fcm gerekli teknik ve idari tedbirleri alma;<\/span><\/li>\n<li><span style=\"color: #000000;\">Size sundu\u011fumuz sa\u011fl\u0131k hizmetlerinin geli\u015ftirilmesi ve iyile\u015ftirilmesi amac\u0131yla sa\u011fl\u0131k hizmetleri kullan\u0131m\u0131n\u0131z\u0131 analiz etme ve sa\u011fl\u0131k verilerinizi saklama;<\/span><\/li>\n<li><span style=\"color: #000000;\">\u0130lgili mevzuat gere\u011fi saklanmas\u0131 gereken sa\u011fl\u0131k verilerinize ili\u015fkin bilgileri muhafaza etme;<\/span><\/li>\n<li><span style=\"color: #000000;\">Anla\u015fmal\u0131 oldu\u011fumuz kurumlarla size sunulan sa\u011fl\u0131k hizmetlerine ili\u015fkin finansal mutabakat sa\u011flanmas\u0131;<\/span><\/li>\n<li><span style=\"color: #000000;\">Hasta memnuniyetinin \u00f6l\u00e7\u00fclmesi d\u00e2hil ancak bunlarla s\u0131n\u0131rl\u0131 olmaks\u0131z\u0131n, benzeri ama\u00e7larla i\u015flenebilecektir.<\/span><\/li>\n<\/ul>\n<p><span style=\"color: #000000;\">Ayr\u0131ca ki\u015fisel verileriniz, 3359 say\u0131l\u0131 Sa\u011fl\u0131k Hizmetleri Temel Kanunu, 663 say\u0131l\u0131 Sa\u011fl\u0131k Bakanl\u0131\u011f\u0131 ve Ba\u011fl\u0131 Kurulu\u015flar\u0131n\u0131n Te\u015fkilat ve G\u00f6revleri Hakk\u0131nda Kanun H\u00fckm\u00fcnde Kararname, \u00d6zel Hastaneler Y\u00f6netmeli\u011fi, Ki\u015fisel Sa\u011fl\u0131k Verilerinin \u0130\u015flenmesi ve Mahremiyetinin Korunmas\u0131 Y\u00f6netmeli\u011fi ve Sa\u011fl\u0131k Bakanl\u0131\u011f\u0131 d\u00fczenlemeleri ve sair mevzuat h\u00fck\u00fcmleri \u00e7er\u00e7evesinde i\u015flenebilecek olup, \u0130stanbul Hospital\u2019a ve\/veya tedarik\u00e7ilerimize ait fiziki ar\u015fivler ve bili\u015fim sistemlerine nakledilerek, hem dijital hem de fiziki ortamda muhafaza alt\u0131nda tutulabilecektir.<\/span><\/p>\n<p><span style=\"color: #000000;\">Ayr\u0131ca, Hastane \u00e7al\u0131\u015fanlar\u0131m\u0131z\u0131n verileri, \u0130\u015f Kanunu ve \u00c7al\u0131\u015fma ve Sosyal G\u00fcvenlik Mevzuat\u0131 ile y\u00fcr\u00fcrl\u00fckte olan di\u011fer mevzuat\u0131n \u00f6ng\u00f6rd\u00fc\u011f\u00fc zorunluluklar kapsam\u0131nda, \u00e7al\u0131\u015fan memnuniyetini artt\u0131rmak ve i\u015f g\u00fcvenli\u011fi ile i\u015f bar\u0131\u015f\u0131n\u0131n sa\u011flanmas\u0131 gibi i\u015fletmesel nedenler ile i\u015fverenin y\u00f6netim hakk\u0131 gere\u011fince taraf\u0131m\u0131zdan veya i\u015fbirli\u011fi yapt\u0131\u011f\u0131m\u0131z veya yetkilendirdi\u011fimiz ger\u00e7ek veya t\u00fczel ki\u015filer taraf\u0131ndan i\u015flenebilecektir. Di\u011fer sa\u011fl\u0131k kurumlar\u0131ndaki tetkik, tedavi gibi s\u00fcre\u00e7lere ili\u015fkin olarak hastanemize teslim etmi\u015f oldu\u011funuz tetkik sonucu, rapor, r\u00f6ntgen gibi sa\u011fl\u0131k verileriniz, taburcu oldu\u011funuzda teslim almad\u0131\u011f\u0131n\u0131z takdirde \u00fc\u00e7 ay i\u00e7erisinde imha edilecektir.<\/span><\/p>\n<\/div>\n<\/div>\n<\/div>\n<div class=\"elementor-element elementor-element-0e55d0c elementor-widget elementor-widget-text-editor\" data-id=\"0e55d0c\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n<div class=\"elementor-widget-container\">\n<div class=\"elementor-text-editor elementor-clearfix\">\n<h3><span style=\"color: #000000;\">2. K\u0130\u015e\u0130SEL VER\u0130LER\u0130N\u0130Z\u0130N AKTARILAB\u0130LECE\u011e\u0130 K\u0130\u015e\u0130 ve KURULU\u015eLAR<\/span><\/h3>\n<p><span style=\"color: #000000;\">Ki\u015fisel verileriniz, Kanun ve benzeri mevzuat kapsam\u0131nda ve yukar\u0131da yer verilen ama\u00e7larla:<\/span><\/p>\n<ul>\n<li><span style=\"color: #000000;\">\u0130stanbul Hospital Sa\u011fl\u0131k Grubu Kurulu\u015flar\u0131,<\/span><\/li>\n<li><span style=\"color: #000000;\">Sosyal G\u00fcvenlik Kurumu,<\/span><\/li>\n<li><span style=\"color: #000000;\">Sa\u011fl\u0131k Bakanl\u0131\u011f\u0131 ve ba\u011fl\u0131 alt birimleri, Aile Hekimli\u011fi Merkezleri<\/span><\/li>\n<li><span style=\"color: #000000;\">\u00d6zel sigorta \u015firketleri (sa\u011fl\u0131k, emeklilik ve hayat sigortas\u0131 ve benzeri)<\/span><\/li>\n<li><span style=\"color: #000000;\">Emniyet Genel M\u00fcd\u00fcrl\u00fc\u011f\u00fc ve benzeri kolluk kuvvetleri,<\/span><\/li>\n<li><span style=\"color: #000000;\">N\u00fcfus Genel M\u00fcd\u00fcrl\u00fc\u011f\u00fc,<\/span><\/li>\n<li><span style=\"color: #000000;\">T\u00fcrkiye Eczac\u0131lar Birli\u011fi,<\/span><\/li>\n<li><span style=\"color: #000000;\">Mahkemeler,<\/span><\/li>\n<li><span style=\"color: #000000;\">T\u0131bbi te\u015fhis i\u00e7in i\u015f birli\u011fi i\u00e7erisinde oldu\u011fumuz laboratuvar, merkez ve benzeri \u00fc\u00e7\u00fcnc\u00fc ki\u015filer,<\/span><\/li>\n<li><span style=\"color: #000000;\">Yetki vermi\u015f oldu\u011funuz temsilcileriniz,<\/span><\/li>\n<li><span style=\"color: #000000;\">Hastan\u0131n sevk edildi\u011fi veya hastan\u0131n kendisinin ba\u015fvurdu\u011fu sa\u011fl\u0131k kurulu\u015fu<\/span><\/li>\n<li><span style=\"color: #000000;\">Avukatlar, vergi dan\u0131\u015fmanlar\u0131 ve denetimciler de d\u00e2hil olmak \u00fczere dan\u0131\u015fmanl\u0131k ald\u0131\u011f\u0131m\u0131z \u00fc\u00e7\u00fcnc\u00fc ki\u015filer,<\/span><\/li>\n<li><span style=\"color: #000000;\">D\u00fczenleyici ve denetleyici kurumlar, resmi merciler<\/span><\/li>\n<li><span style=\"color: #000000;\">Hizmetlerinden faydaland\u0131\u011f\u0131m\u0131z veya i\u015fbirli\u011fi i\u00e7erisinde oldu\u011fumuz tedarik\u00e7ilerimiz, destek hizmet sa\u011flay\u0131c\u0131lar\u0131m\u0131z ve i\u015f ortaklar\u0131m\u0131z ve di\u011fer \u00fc\u00e7\u00fcnc\u00fc ki\u015filer ile KVKK\u2019nun 8. ve 9. maddelerinde belirtilen ki\u015fisel veri i\u015fleme \u015fartlar\u0131 ve ama\u00e7lar\u0131 \u00e7er\u00e7evesinde aktar\u0131labilecektir.<\/span><\/li>\n<\/ul>\n<\/div>\n<\/div>\n<\/div>\n<div class=\"elementor-element elementor-element-d1a62f4 elementor-widget elementor-widget-text-editor\" data-id=\"d1a62f4\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n<div class=\"elementor-widget-container\">\n<div class=\"elementor-text-editor elementor-clearfix\">\n<h3><span style=\"color: #000000;\">3.
